3.) ASAP after that, we will e-mail you your Unique Identifier.

3.) When you receive that, and print your information, mail a copy of it and the unique identifier to the snail mail address you will be given.

I, the coach or manager of the above team, assure that all information is correct and also understand that if my team is not accepted, the entry fee will be returned in full. I further understand that once my team is accepted and later withdraws, the entry fee is forfeited. No refund will be made in the event of cancellation or shortening of any matches due to inclement weather. I agree to abide by all the USYSA and OYSAN tournament rules.
